The driver involved in the Chattanooga, Tennessee, school bus accident that left 5 children dead was traveling "well above" the speed limit of 30 mph, according to an affidavit.

Moments after the catastrophic crash on Monday, the driver, Johnthony Walker, called his mother and told her what happened.

" 'Mama, I love you. I have been in a drastic accident,' " Gwenevere Cook recalled her son saying. "He texted me minutes later saying the kids are dead."

Twelve other children were injured.
